Of the 1,400 forms we mailed out, less than half were returned. If you still have not returned your form, please take the time to complete it and return it. If you cannot find the form we sent or did not receive one, just click here. Copies may also be found in a basket located in the parish hall labeled Parish Census Forms. Simply complete the form and place it into the collection bag during Mass or mail/deliver it to the rectory.

Our parish is required by the archdiocese to have contact data for each of our members. This means names that are spelled correctly, addresses that are current and telephone numbers that are not disconnected. So, please take your time and print legibly. It will help us out tremendously when we enter the information. It does not matter if your information has not changed since last year. We still need to verify that you are an active member by having you return this form to us. Unfortunately for us, we do not have current phone numbers of many of the parishioners who have not returned the census form.
